You must be appointed with an insurance carrier before an application can be taken. Carolyn and Felicia will work closely with you and each of the carriers to ensure your appointment goes smoothly. Your marketer should provide you with the appropriate contracting forms for the carriers you wish to represent or you can download them. We attach a checklist and attached it to each of the carriers’ kits so you’ll know exactly what is required to process your appointment.
Simply fax your completed forms along with all other required documents (e.g. state insurance license, E&O certificate) to our licensing department at 781-740-9778. Upon receipt of your forms, one of our licensing administrators will contact you to confirm receipt and advise if your forms are “in good order” or if additional information is required.
We will submit your forms to the carrier then notify you once your appointment has been approved and provide you with your new agent code. It is important to note that if for some reason your appointment is denied, our licensing administrators will work with you to resolve the problem.
If you are currently appointed with a carrier through another organization it is still possible to transfer your appointment to dmi Although each carrier’s rules regarding transfers vary, most allow a transfer if you have not submitted any business within a specified time-frame (e.g. six months). In that situation, no action is required on your part other than to follow the appointment process outlined above. If you do not meet the carrier’s guidelines for a transfer, you will be required to obtain a “release” from your current marketing organization. If this is required, please work with your account executive at dmi who will work with you and your current marketing organization.
IMPORTANT NOTICE! If you are currently with a Broker Dealer that requires your FIA business to be conducted through the BD you may need different L&C paperwork.
